The purpose of our health services program is to protect our children from disease and to administer first aid. The program keeps both our teachers and parents aware of the health needs of the children.
Within Glenview School District 34 there is one certified school nurse who helps coordinate our school health services. There is one health coordinator at each school who maintains student health records, cares for the injured or ill, administers and records medication, screens vision and hearing, monitors health restrictions and provides continuing education to school staff.
School District 34 enforces Illinois health requirements. These requirements consist of health examinations, immunizations, dental examinations and eye examinations specific to grade and age.
Physical Examinations
The state of Illinois requires a health examination
- prior to beginning an early childhood program
- prior to entering kindergarten
- prior to entering sixth grade*
- prior to entering ninth grade
On August 24, 2007, the Illinois General Assembly passed a law (Public Act 95-422) amending the physical examination requirement in the Child Health Exam Code. The amendment changed the date of the required health examination from "prior to entering 5th grade" to "prior to entering 6th grade." This means current fourth grade students will not be required to receive a physical before they enter 5th grade. Students who received a health examination prior to entering 5th grade during the 2007-08 school year will not have to receive another exam before entering the sixth grade in the 2008-09 school year.
Lead screening is also a required part of the health examination for children age six years or younger prior to admission to kindergarten or first grade. Each parent or legal guardian shall provide a statement from a physician or health care provider that the child has been risk assessed if the child resides in an area defined as low risk or screened for lead poisoning if the child resides in an area defined as high risk. (Section 7.1 of the Lead Poisoning Prevention Act)
In addition, annual physical examinations are required for interscholastic sports participation.
Students who do not show proof of the physical exam and immunizations will not be allowed to start school.
Dental Examinations
Dental exams are required for students entering kindergarten, second and sixth grade.
Eye Exams
The State of Illinois now requires all children, beginning kindergarten for the first time or students who are transferring into Illinois from out of state, to have a comprehensive eye examination prior to starting school. The examination must be done by an optometrist or ophthalmologist. The results must be submitted to the school on a form approved by the Ilinois Department of Public Health (see below). This requirement begins with the 2008 school year and applies to all future years.
Additional Information
Physcials are due by the 1st day of school for:
* Early Childhood/Preschool
* Kindergarten
* Sixth Grade
Students who do not show proof of the physcial exam will not be allowed to start school.
Sports Physicals are due every year for participation in interscholastic sports.
Immunizations do not need to be complete until October 15th.
Dental Exams are due by May 15th of current school year for
* Kindergarten
* 2nd Grade
* 6th Grade
Eye Exams are due by October 15th for
* Kindergarten
* Any student enrolling for the first time in an Illinois school in kindergarten or above
