District 34 policy requires that students in grades Kindergarten and sixth, and those who are new to the District, submit the appropriate forms indicating that they have all immunizations up to date and a physical within the last calendar year. Students who do not have these forms on file will not be able to start school. Learn more about the requirements, and find copies of health forms, on the District’s website. 

You can upload completed forms quickly, easily and securely any time.

Or, you can drop off completed forms with your doctor’s signature at Attea Middle School, 2500 Chestnut Ave, on Tuesday, Wednesday or Thursday from 8:30am- 2:00pm.

Before you submit forms, make sure to complete and sign the Health section of your child's physical form. It is not valid unless this section is completed and signed!
